Transportation Change Form
This form must be filled out upon initial enrollment and when there is a PERMANENT CHANGE to a student's pick-up or drop-off location. Please Note: Student information is retrieved from the district's student database, Infinite Campus. Bus stop assignments are based on the information retrieved from this database. A permanent change in residency can only be made through the school - DO NOT USE THIS FORM TO REPORT A CHANGE OF ADDRESS. The Transportation Department cannot take this information from a parent/guardian. Transportation will be provided after this change is made in Infinite Campus.
To ensure this change is in compliance with our transportation policies, please contact the Transportation Department (207-929-9153) within 2(two) days of submitting the form.
